**You are required to bring your insurance card with you to each and every visit. FPC reserves the right to refuse service to any
individual who cannot produce his/her current
insurance card**

Our office desires to keep the cost of your medical care as low as
possible. To do this, you must assume responsibility for payment of
services rendered.

Co-payments are expected to be paid when you check in with our front desk on the day of your appointment.

Once we receive payment from your insurance company, a
statement will be sent to your home for any remaining balances.
Payment must be made within 30 days or arrangements can be made with the billing department.

Our office is contracted with many different insurance plans, PPOs, HMOs and managed care providers as a convenience to
patients within our community. It is in your best interest to
understand your individual plan.

We are currently contracted with Unison Health Plan for the Ohio
Medicaid Managed Care Plans. We cannot accept any other managed care plans at this time.
